- “Efficiency” projects fail on the audit question
- The build goes well until someone asks how a decision would be explained to a regulator eighteen months later. If that question has not been designed for, the answer is usually a rollback. We ask it in the first week instead.
- Core modernisation is a sequencing problem
- The technology is rarely the hard part. Deciding what moves first, what stays, and how the two coexist for the eighteen months in between is where programmes are won or lost.
- Document AI needs a confidence threshold, not just accuracy
- A model that is right ninety-five per cent of the time is only useful if it reliably knows which five per cent to hand to a person. Designing that handoff matters more than the last point of accuracy.
- Resilience testing is usually theoretical
- Recovery plans exist; they have often never been run end to end under realistic conditions. The gap between the document and the drill is where the actual risk lives.
